Abstract: Aim and objectives The present study is undertaken to assess the role of salivary levels of Interleukin-21, Ig A, C-reactive protein and bicarbonate ions as good indicator in chronic periodontitis patients .The objective was to evaluate serum and salivary levels of Interleukin -21and salivary Ig A in periodontally healthy individuals and chronic periodontitis patients. Also evaluate C-reactive protein and inorganic bicarbonate ions in saliva and serum. And establish the correlation between them. These markers can be used as good indicator of chronic periodontitis. Material and Method In the present case control study, total 100 participants were included. Group-1 comprises 50 periodontally healthy controls, Group-2 newly diagnosed chronic periodontitis patients. The biochemical parameters IL-21, CRP, HCO3- measured in serum and saliva and salivary Ig A. Serum and salivary IL-21, salivary Ig A, and CRP estimated by ELISA technique. The serum hs CRP and serum and salivary HCO3- assessed on Dade dimension .The results were analyzed with appropriate statistics using SPSS 20.0 software. Results Serum and salivary IL-21, CRP, and HCO3- were considerably increased in CP cases when compared to periodontally healthy controls. Also salivary Ig A levels increased in saliva of CP cases as compare to healthy controls. The statically positive correlation in CP and periodontal healthy controls for serum and salivary IL-21,CRP and Ig A. Clinical periodontal parameters correlated positively with serum and salivary IL-21 levels, salivary CRP and salivary Ig A. With the help of ROC curve best cutoff values detected for all study parameters in CP patients. Conclusion Salivary IL-21, Ig A can be used as good indicator in CP patients. Salivary CRP is used to find out inflammation severity in oral disease other than serum. Also there is role of HCO3-in disease progression in CP. Hence it may conclude that, inflammatory markers evaluated from saliva may use for diagnosis and prognosis of periodontitis instead of serum marker evaluation
