Background: Chronic inguinal neuralgia is one of the most significant complication following inguinal hernia repair. Routine ilioinguinal neurectomy has been proposed as a means to avoid this complication. The purpose of this report is to evaluate the effect of ilioinguinal neurectomy on chronic groin pain following open mesh hernia repair compared with ilioinguinal nerve preservation. Materials And Methods: In this prospective randomized study 56 patients undergoing open mesh repair for inguinal hernia are divided into two groups . In Group A , ilio inguinal nerve preserved and in Group B ilio inguinal nerve excised . Postoperative chronic groin pain and wound complication are compared between the two groups. The results are finally analyzed and compared for the two groups using Chi Square test. Results: There was reduction in the intensity of groin pain in the ilio inguinal nerve excised group than the preserved group till 6 month but the there was no significant difference in groin pain from 6 month and post operative hypoesthesia was more in nerve excised group and complications like seroma, are comparable in two groups.
