Abstract: | AIMS & OBJECTIVES: To evaluate D-dimer levels in predicting the severity of acute pancreatitis .To evaluate the ability of the D-dimer to assess early development of organ failure. MATERIALS AND METHODS: This is a prospective observational study of 115 patients presented with Acute Pancreatitis in B.L.D.E (DU)’S Shri B.M.Patil Medical College and the D-dimer values for these patients were measured on day 1, 3, 5. RESULTS: Out of 115 patients of acute pancreatitis 31-40 years age group was predominant (20.6 %). Majority were the male patients (91.3%) in the study. Gall stones is the major etiological factor observed in the patients (62%). Age of the total patient was compared with raised d-dimer values using spearman’s and was considered significant. Common modes of presentation in this study were pain abdomen (85%), vomiting (27%). D-dimer values measured on admission day and subsequently on day 3 and 5 showed increased association with acute pancreatitis, as well as predicted early the occurrence of organ failure. Diagnostic accuracy and ROC analysis of D-dimer score was compared with the remaining three scoring systems that is APACHE 2, SOFA, Ranson’s score were (p less than 0.001). It has a sensitivity of 94% and specificity of 86.2%. This is much higher than the sensitivity and specificity compared with other three studies. CONCLUSION: D-dimer value is a useful tool for Diagnosis of acute pancreatitis as well as predicting the occurrence of organ failure early.
